What is Coordinated Career Pathways?

Coordinated Career Pathways (CCP) is a new service designed for individuals to achieve their career goals. It helps individuals find jobs in the community that offer equal pay, benefits, and growth opportunities. CCP services will be tailored to the specific needs and work goals of the individual.

Tailored Day Services (TDS)

 

On the fence about a job? TDScan help you find employment, get out in the community for activity, work on your job skills, volunteer to gain work experience, and help with looking into your education.

Supported Employment (SE)

 

Are you ready for a job in the community? Do you need onsite support to learn your job duties or help requesting accommodations? SE would be a great fit for you! A job coach can help!

Paid Internship (PIP)

 

Do you need work experience? A Paid internship is a great way to learn new job skills and get some experience to build your resume for future jobs. Not your forever job, just a jumping point!

Coordinated Career Pathways (CCP)

 

If you are exiting a work activity program, subminimum wage setting, or within two years of exiting high school to achieve or advance in employment with a Career Pathway Navigator (CPN) and a Customized Employment Specialist (CES). Services are time- limited to 18 months but can be extended to a maximum of 24 months.

Day Program

 

Not ready to work yet? Or, maybe you need a break from working! Day programs are a great way to engage with your peers and with the community during your day. You can be onsite or out and about doing activities. You can always explore employment at your own pace when you are ready!

It is up to you! You are the captain of this ship! There are so many options available to help you achieve your goals! Talk with your SC to see which option if best for you.

Two new services are available through CCP!​​

Career Pathways Navigator (CPN)

CPN helps to navigate the individual and their families through career planning and identifying career goals. CPN will work with you and your family to create a Person-Centered Career Plan.​

Customized Employment Specialist (CES)​

CES helps the individual secure a job customized to their unique talents, skills, and interests that match the employer’s needs.

 

The CES will expand on initial information from the PCCP to direct, assist, and support the individual through an individualized, person-centered, customized employment process.

Other Services and Coordinated Career Pathways

CCP can be used together with other services including Supported Employment (SE), Community Integration Training (CIT), and Tailored Day Services (TDS). Example: Career Pathway Navigator (CPN) is in place and in the PCCP it’s identified that SE can/should be used to assist with obtaining employment and provide job coaching.

 

CCP does NOT include job coaching. Other services, like SE or TDS, would have to be used for job coaching.

How Long Can I Get This Service?

Participants can receive this service for 18 months. It may extend to 24 months based on the individual’s needs (contact your Service Coordinator).

CCP is not a full-time service

Who Is
Eligible
For This Service?

  • Individuals who are exiting a work activity program(s)​

  • Individuals who are exiting a program where they are earning less than minimum wage

  • Individuals who are enrolled in special education and is within 2 years from transitioning from secondary education, including those who have chosen to exit secondary education​

  • Participants of the Self Determination Program (SDP) can access CCP who meet the CCP service eligibility criteria - learn more

What Is 
Required
For This Service?

All individuals using Coordinated Career Plan are required to have a Person Centered Career Plan. The written plan includes the individual’s career interests and goals, strengths, challenges and barriers, regional center and generic services and supports, short-term milestones, action steps, and timeline.

 

A PCCP does not automatically assume an individual will go into customized employment. An individual will seek customized employment if it fits the needs and wants of the individual based on the desired outcomes of the Person-Centered Career Plan. 

How can I 
Receive
This Service?

Talk to your Service Coordinator to request the service. 

The process begins with the Service Coordinator completing the Referral and Service Need Evaluation form to determine eligibility for services. This form is then reviewed and approved by the San Diego Regional Center (SDRC) Review Team to confirm if it meets eligibility requirements. Once approved, the Service Coordinator forwards the referral form along with any additional supporting documents, such as the Individual Program Plan (IPP), Individualized Education Program (IEP), or pertinent assessments, to the designated vendor.

If customized employment is identified as a goal in the Person-Centered Career Plan (PCCP), the individual will collaborate with a Customized Employment Specialist (CES). The CES works closely with both the individual and potential employers to create tailored job opportunities that align with the individual's unique skills and interests. This may also include exploring self-employment or microenterprise options. Throughout the process, the CES monitors progress toward the PCCP goals and works jointly with the Career Pathway Navigator (CPN) to develop transition plans.

The CPN plays a key role in guiding the individual through their career path by connecting them with necessary services and supports. These services may include options such as college, vocational training, supported employment, or participation in paid internship programs. A comprehensive PCCP is collaboratively developed by the CPN and the individual, outlining specific milestones and strategies to achieve career goals. This plan is then shared with the Service Coordinator, who reviews it with the IPP team to initiate access to the identified services, including customized employment if applicable.

The CPN continues to meet with the individual regularly to assess progress, address any needs for adjustments to the PCCP, and ensure milestones are being achieved effectively. This collaborative effort ensures that the individual receives personalized support and opportunities tailored to their career aspirations.
